Dear Current and New Clients for TAS (ADA Related) Services,

Before a Fokus RAS can accept your documents for Review and/or Inspection, the following must be provided:

  1. Payment in Full - all paid fees are non-refundable. Our fee schedule is available for download. Make checks payable to Fokus on Architecture, Inc. We do not have credit card capabilities at this time. For any questions regarding payment, please call the office.

  2. Provide the completed Project Registration Form, fully filled out to include CAD (County Appraisal District) Number and Printout.
    *Please note that the Owner or Design Professional can register the project on-line, in which case the Registration Confirmation page needs to be submitted to the RAS along with all other documents.

    NOTICE: All registered projects will now require the County Appraisal District (CAD) number and the CAD document to be uploaded to TABS. Along with the CAD number and document, the Limited Liability Ownership (LLO) form will be required to be completed in full and uploaded to TABS when the building owner is a Limited Liability Corporation (LLC), Limited Liability Partnership (LLP) or Limited Partnership (LP). With an LLC, LLP or LP, a copy of the Articles of Formation (AOF) or copy of the Texas Secretary of State (SOS) document must also be uploaded.

    Complete and accurate documents are required by the state website at the time of registration and no TABS number can be provided without all of the information submitted to the RAS. If the RAS determines that the forms have been filled out inaccurately they will be returned until filled out correctly before attempting registration.   3. Complete the Proof of Submission Form, signed and dated. Provide a copy of the completed form with a digital copy of the construction documents - (signed and sealed Construction Documents ready to be issued for submission to regulatory agencies for approval Permit, Construction, or public bidding. Do not submit preliminary drawings for budget pricing). Hard copies of partial plans may be requested by the RAS to assist in the review process.
    Please note that TDLR may fine the Design Professional, if you do not submit your documents for TAS Review within 20 business days of the “Issue” date, as defined on the form. All the dates must be filled out on the form for it to be considered valid for submission.

  4. Unless the Owner (who holds title to the property) submits the project themselves and is available to sign the subsequent documents, the party registering the project must provide a completed Designation Agent Form, signed and dated. This form may need to be filled out more than once if more than one entity is being asked to sign on behalf of the owner. Any employee of the ownership entity may sign this form.   5. Request for Inspection Form - We strongly suggest this form be submitted along with all the other forms at the time of registration to complete the paperwork process as accurately as possible with all the matching signatures required. Fokus is not responsible if project closure is denied by the state because this paperwork is missing or incomplete at the end of the project. Missing information and denial of closure could result in a state fine for the property owner if it is not resolved within 1 year of the construction completion date on the registration form.

Project Submissions which are missing any of documents and information CANNOT be processed in the program. It is not possible to leave information blank and therefore not possible to give you a TABS number. We are not responsible for lack of information or incorrect information resulting in delays for obtaining city permits because they require the TABS number. Any changes to property ownership between registration and inspection of the project will require new forms and should be brought to the RAS’s attention before the inspection date.
